Janumet 50/500mg Tablet is used alongside diet and exercise to control blood sugar in adults with type 2 diabetes when a single medicine is not enough. It contains sitagliptin and metformin, which lower glucose through two separate mechanisms.
Metformin reduces the glucose the liver releases and improves how well muscle responds to insulin. Sitagliptin blocks the enzyme DPP-4, protecting the gut hormones that prompt the pancreas to release insulin after a meal and reduce glucagon, so insulin is released only when glucose is actually rising.
Take it twice daily with or immediately after food, which substantially reduces the nausea, metallic taste and loose stools metformin can cause. Do not stop when your readings improve; that improvement is the treatment working.
Seek urgent medical attention for severe persistent upper abdominal pain spreading to the back, which may indicate pancreatitis, or for unusual muscle pain, rapid breathing and extreme tiredness, which may indicate the rare complication lactic acidosis.
Kidney function must be checked before starting and at least yearly, and the medicine stopped before contrast scans and surgery. Long-term metformin lowers vitamin B12, so levels should be monitored periodically.
